The workload of US radiologists has increased over the past two decades.1 <\/sup>However, the number of new radiologists entering the field hasn\u2019t grown to match demand. This shortage, combined with increasing demand for imaging services, has drawn radiologist productivity measurement and management back into the spotlight. <\/p>\n\n\n\n

In this blog, we\u2019ll examine why standard productivity tools like relative value units (RVUs) miss the mark for radiology services, and how a digital provider scheduling solution<\/a> can supplement RVUs to improve measurement while enhancing radiology productivity.<\/p>\n\n\n\n

Misreading Productivity: Limitations of RVUs in Radiology<\/h2>\n\n\n\n

In medicine, RVUs are a measure of productivity frequently used to \u201csupport the calculation of physician reimbursements.\u201d Many radiology practices and departments use RVUs for this purpose, but this system has known limitations.2<\/sup> The RVU concept was first conceived by a Harvard economics professor in the 1980s,3 <\/sup> and over time, they\u2019ve become instrumental in defining physician compensation.<\/p>\n\n\n\n

  1. RVUs are incapable of accounting for demand fluctuations expected across radiologist shifts (e.g., prime-time day shifts compared to night and weekend shifts that are less reliably busy). <\/li>\n\n\n\n
  2. RVUs ignore capacity limitations driven by finite or underutilized equipment.<\/li>\n\n\n\n
  3. RVUs only consider revenue-generating productive work. They fail to account for non-revenue generating work like conferences, meetings, training, documentation, and other administrative assignments. This effectively penalizes radiologists for investing in professional development and broader organizational needs.<\/li>\n\n\n\n
  4. RVU definitions vary across imaging specialties and modalities, making it nearly impossible to compare productivity across sub-specialties.<\/li>\n<\/ol>\n\n\n\n

    Because RVUs fail to provide insight into all of the work providers are doing, and unless your team equally distributes holidays, weekends, and other shifts, RVUs can\u2019t reliably measure radiologist productivity.<\/p>\n\n\n\n

    To make a long story short, RVU metrics don\u2019t accurately tell us how much work physicians are doing. Using the logic that underpins RVUs, any time a physician is not \u201cproducing,\u201d it\u2019s considered \u201cwasted time.\u201d<\/p>\n\n\n\n
